Referral Authorization Form

No FWS student employee may begin work until he/she has obtained an FWS referral authorization form (Sample) from the Office of Student Financial Services. The authorization form identifies the student as a participant in the FWS Program and the amount awarded. Actual earnings will depend on the number of hours worked and the hourly rate of pay.

Please note: Students may work in more than one department at a time and may be utilizing or have already exhausted a portion of the FWS award indicated on the authorization form.

The FWS authorization form must be signed by the student and the employer. The hiring department must forward the authorization form to Student Employment Services, noting the reference number (Doc #) of the student's Electronic Payroll Form. Failure to submit this form along with a Electronic Payroll Form will result in the delay of the payroll process.
